Chronic Neck Pain (WALT Standard)
Source: WALT Guidelines 2010
WALT protocol for chronic neck pain targets the cervical paraspinal muscles, trigger points, and facet joints. Near-infrared wavelengths penetrate through the skin and muscle layers to reduce myofascial tension, nerve inflammation, and pain signaling.
Protocol Parameters
| Condition | Chronic Neck Pain |
| Wavelengths | 810nm, 830nm, 850nm |
| Dosage | 4 J per point, 6-8 points |
| Irradiance | 50-100 mW/cm² |
| Duration | 30-60 seconds per point |
| Frequency | 3x per week for 3-4 weeks |
| Body Zone | neck |
| Evidence Level | Strong |
Mechanism of Action
NIR photons are absorbed by cytochrome c oxidase in mitochondria of muscle cells and neurons, reducing oxidative stress, increasing ATP production, and lowering inflammatory mediators. This reduces muscle spasm, improves local circulation, and decreases pain nerve sensitization.
Key Studies
- Chow et al. (2009) - Lancet systematic review confirming efficacy
- Gross et al. (2013) - Cochrane review supporting PBM for neck pain
- Alayat et al. (2014) - RCT showing PBM superior to placebo and exercise alone
Treatment Tips
- *Target trigger points and tender points specifically
- *Include the upper trapezius and levator scapulae insertion
- *Consider treating from C3-C7 bilaterally
- *Combine with gentle stretching and ergonomic modifications
Contraindications
- !Over the thyroid gland (use caution)
- !Active cancer in cervical region
- !Carotid artery disease (avoid direct application over carotids)
