Knee Osteoarthritis (WALT Standard)
Source: WALT Guidelines 2010
The World Association for Photobiomodulation Therapy (WALT) protocol for knee osteoarthritis uses near-infrared wavelengths applied to specific points around the knee joint. This protocol targets the medial and lateral joint lines, suprapatellar pouch, and pes anserinus insertion to reduce inflammation and pain while promoting cartilage health.
Protocol Parameters
| Condition | Knee Osteoarthritis |
| Wavelengths | 810nm, 830nm, 850nm |
| Dosage | 6 J per point, 4-6 points |
| Irradiance | 50-100 mW/cm² |
| Duration | 30-60 seconds per point |
| Frequency | 3x per week for 4 weeks, then maintenance |
| Body Zone | knee |
| Evidence Level | Strong |
Mechanism of Action
NIR light penetrates to the synovial membrane and articular cartilage, reducing inflammatory cytokines (TNF-alpha, IL-1beta), stimulating chondrocyte activity, and increasing local blood flow. The anti-inflammatory effect reduces swelling and pain while supporting tissue repair.
Key Studies
- Bjordal et al. (2003) - Systematic review showing significant pain reduction
- Hegedus et al. (2009) - RCT showing improved microcirculation and reduced pain
- Alfredo et al. (2012) - Combined PBM with exercise showed superior outcomes
Treatment Tips
- *Apply to 4-6 specific points around the knee, not broad-area treatment
- *Combine with appropriate exercise program for best results
- *Continue maintenance sessions even after pain reduction
- *Use higher wavelengths (810-850nm) for joint penetration
Contraindications
- !Active cancer in the treatment area
- !Over infected joints
- !During acute flare-ups (wait 24-48 hours)
